Scope of Position

The Ohio State University Comprehensive Cancer Center – James Cancer Hospital’s mission is focused on integrating quality patient care, education, teaching, and research. The Nurse Manager-Continuing Care is a visible leader for the areas of continuing care including James Clinical Call Center Triage RN, post-acute care (PAC), remote monitoring, and home-based care.  The Nurse Manager-Continuing Care will have direct oversight over the RNs and staff in these areas to ensure execution of programs, projects, related facilities, information technology, and compliance to ensure appropriate care delivery process while cultivating collaboration, enthusiasm, and mutual respect. 

Position Summary

The Nurse Manager-Continuing Care has 24-hour responsibility for the planning, organization, implementation and evaluation of the James Clinical Call Center Triage RN (24/7), PAC, remote monitoring, and home-based care. The Nurse Manager-Continuing Care is key in operationalizing the Cancer Program’s mission, values and goals and creates an environment that supports the nursing professional practice model.  

The Nurse Manager-Continuing Care is responsible for managing, guiding, coaching, and evaluating the work performance of the continuing care staff including the James Clinical Call Center Triage RN, PAC, remote monitoring, and home-based care teams. The Nurse Manager-Continuing Care is responsible for executing the continuing care programs within the Outpatient Care Management department and collaborating with other areas in the Integrated Care Management department as well as other areas within the James and the Health System. The Manager-Continuing Care will assist in the development of innovative care management models and approaches in the outpatient setting in alignment with the organization’s value-based care and population health strategies.

Minimum Requirements

Bachelor’s degree in nursing required.  Master’s Degree in a related field of study required. 5+ years acute care hospital and/or post-acute care experience preferred. Care coordination, RN triage, utilization, case/disease management experience preferred. Data analytics and reporting experience preferred.  Experience in Value Based Care models, managed care, customer service, and provider relations preferred.  Proficiency with Microsoft Office applications, specifically Excel and PowerPoint required.

  • Significant understanding of the care continuum and health care provider organizations – acute and post-acute care facilities and providers.
  • Experience in project management.
  • Solid time management/problem solving skills.
  • Experience working effectively with a wide range of colleagues, from entry level to senior executives, physicians, nurses, administrative and information technology staff.
  • Strong verbal and written communication, organizational and human relations skills, self-motivated, professional.
