Scope of Position:
Case Management is a team process that includes assessment, planning, facilitation, and evaluation of the services required to meet the patient’s needs through the healthcare system. Social work services are provided to patients/families to meet their medically related social and emotional needs which impact their medical condition, treatment, recovery, and safe transition from one care environment to another.
Position Summary:
The Case Management Extender will provide programmatic support services for the Case Management & Social Work areas covering patients at The James Cancer Hospital. The Program Coordinator will be responsible for coordinating PCRM/SW program support inclusive of discharge plan implementation, prior authorization, scheduling, data entry and other PCRM/SW extender functions at the direction of the Patient Care Resource Manager and/or Social Worker for selected teams.
Minimum Qualifications:
Bachelor’s degree or equivalent experience required.
Experience in Acute Care Hospital setting preferred.
Experience in assisting with patient care operations preferred.
Proficient in Microsoft Office (Word, Excel, Power Point, Outlook).
Experience with EPIC preferred.
Strong verbal and written communication, organizational and human relations skills, self-motivated, professional (Customer service skills).
Ability to prioritize workload.
