Scope of Position This position is responsible for the coordination of follow-up care provided to heart and lung transplant recipients after they have received a transplant.
Position Summary The Comprehensive Transplant Center and all clinic locations are the point of entry for all transplant customers to The Ohio State University Wexner Medical Center. This position functions to assist with coordination of all aspects of post transplant follow-up care. Staff member functioning in this role will act as liaison between transplant surgeons/physicians, nurses, pharmacies, laboratories, home health agencies and other support services needed. Will provide patient education as indicated. Must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age and acuity of the patients being served. Must possess the ability to assess data reflective of the patient’s status and formulate an appropriate course of action in collaboration with transplant team.
Minimum Qualifications
For Hire: Graduate from an approved school of nursing. Current Ohio License. BSN preferred. Prior transplant experience preferred. Current Certified Clinical Transplant Coordinator (CCTC) certification preferred. Flexible, sensitive and adaptable to transplant activities.
On going: Maintain current Ohio Licensure, maintain CPR certification, and maintain CCTC certification. Must be customer service oriented, dependable, self-directed, independent, organized and detail-oriented.
