SCOPE OF POSITION
In cooperation with other pharmacists and pharmacy technicians, acts as a resource and provides answers to drug information questions for cardiology patients, family, pharmacists, nurses, physicians, advanced practice providers, medical students, pharmacy students and other associate hospital personnel. Rounds with medical teams, approves new drug orders, screens for drug-drug, drug-herb/dietary supplement, drug-allergy, drug-food (in cooperation with the Dietetics Department), drug-disease state and drug-laboratory interactions as applicable. Participates in appropriate hospital and/or departmental committees relevant to practice area.
POSITION SUMMARY
This position functions to coordinate all aspects of patients' pharmaceutical needs in the Ross Heart Hospital. Duties include but are not limited to: medication order approval and distribution; assurance of appropriate dose, route, frequency and duration of therapy; patient medication profile review for drug-drug interactions, duplication of therapy, drug allergy interactions and appropriate regimens; coordination of patients' pharmaceutical needs with physicians and nurses. Patient population includes adult, adolescent, and geriatric patients depending on the practice location. Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must be able to demonstrate medication dosing and identify pharmaceutical needs relative to the disease process and age. This position is also responsible for supervising Pharm.D. students, pharmacy residents, technicians, undergraduate and graduate student teaching, and providing educational programs for hospital staff (providers, nurses and pharmacy staff).
Minimum Qualifications
Pharm.D. Degree or B.S. Pharm and equivalent combination of education and experience. Registered pharmacist in the State of Ohio; board certified preferred; advanced degree (Pharm.D./MS) and residency; and/or equivalent experience preferred (e.g. PGY1 and PGY2 residency).
