Scope of Position
In cooperation with physicians, advanced practice providers, nursing, and other health care providers, acts as a resource and provides answers to drug information questions pertaining to OPAT and complex oral antimicrobial therapy. Participates in the ambulatory management of patients receiving OPAT and/or complex oral antimicrobial therapy for patients under the care of the pharmacist. Participates in appropriate hospital and/or departmental committees relevant to the practice area, specifically antimicrobials.
Position Summary
This position functions to coordinate all aspects of patients' pharmaceutical needs related to OPAT and complex oral antimicrobial therapy. Duties include but are not limited to: proactively participating in patient visits; assurance of appropriate dose, route, frequency and duration of therapy; patient medication profile review for drug-drug interactions, duplication of therapy, drug allergy interactions and appropriate regimens; ordering and monitoring appropriate labs; pharmacokinetic monitoring; participates in the transition of care rounds; provides antimicrobial therapy pharmacotherapeutic consults; assists in ensuring patient access to medications; and interactions with specialty pharmacies and home infusion pharmacies as applicable. Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must be able to demonstrate medication dosing and identify pharmaceutical needs relative to the disease process and age. This position is also responsible for supervising pharmacy students, and pharmacy residents, and providing educational programs for patients and hospital staff (providers, nurses, medical students, graduate medical education, and pharmacy staff) as applicable.
Minimum Qualifications
Bachelor's Degree in Pharmacy. Registered Pharmacist. 0 years of relevant experience required. 0-2 years of relevant experience preferred.
