Scope of Position

Acts in an interdisciplinary setting to provide quality ambulatory care to all patients. Collaborates with pharmacists, nurses, physicians, medical students, undergraduate and graduate pharmacy and students and other associate hospital personnel to ensure proper medication standards are being met. Creates and implements policies and procedures regarding medication use, storage, and administration at various sites. Attends meetings with clinical staff to discuss medication-related issues. Acts as a resource and provides answers to drug information questions. Assumes leadership role in identifying evidence-based opportunities for improvement in patient care and organizational processes. Participates in appropriate hospital and/or departmental committees relevant to practice area.

Position Summary

This position functions to coordinate all aspects of patients' pharmaceutical needs in the patient care areas. Duties include but are not limited to: managing all aspects of pharmacy ambulatory services provided, ensures regulatory compliance in ambulatory setting, develops and implements new ambulatory pharmacy services, ensures fiscal viability of programs, assists with indigent care program. Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must be able to demonstrate medication dosing and identify pharmaceutical needs relative to the disease process and age. This position is also responsible for supervising Pharm.D. students, technicians, undergraduate and graduate student teaching, and providing educational programs for interdisciplinary staff (physicians, nurses and pharmacy). Minimum Qualifications
Registered pharmacist in the State of Ohio; board certified; advanced degree (Pharm.D./MS) and residency; and/or equivalent experience preferred. Ability to perform point-of-care INR.
